ADMISSION REQUIREMENT FOR PCC

The admission process is based on appropriateness by a physician’s evaluation and the State of Georgia. The resident must be able to assist with activities of daily living (dressing, mobility, transferring, feeding). The resident must be free of tuberculosis. A current medication list, resident’s history, and physical are required by the State of Georgia prior to admission. Residents with bedsores and/or body tubes are not accepted. Pre-planning is recommended for bed availability, and all admission paperwork must be completed prior to the resident’s arrival. The resident’s insurance information, and a photo ID are required. The family is responsible for supplying personal care items including incontinence care products.

PCC (Personal Care Center) Admission Process
The care of your loved one is very important to us at the Mount Vernon Towers PCC department. In accordance to the State of Georgia regulations, there is an admissions process that must be followed. The admission process starts on the day your loved one is admitted to a hospital and/or rehab facility. It is important for you to contact the director if you are considering admission for respite.
Admission to PCC depends on the resident being appropriate for the setting, and availability of a room. Several factors are evaluated which includes the status of the following: the ability to walk or use an appropriate assistive device, bathing, dressing, eating, grooming, skin integrity, toileting, and transferring. These factors are based on the resident’s independence and needing supervision or requiring assistance. Bedbound residents are not appropriate upon admission. Residents must be able to assist with their activities of daily living.
A current medication list must be on hand prior to admission. The resident must be free of TB. This can be verified by a screening for TB with a skin test or a chest x-ray stating no evidence of TB. The resident may need to sign up with our facility’s pharmacy which is Guardian Pharmacy. (This pharmacy is not a part of Guardian Home Health). We cannot accept prefilled pill boxes. The admission paperwork must be completed, and include a copy of insurance/ID. A copy of a living will or advance directive is needed upon admission.
